Disciplinary Information Statement

 

 

Colorado law requires school principals or their designees to communicate disciplinary information concerning any student enrolled in the school to any teacher who has direct contact with the student in the classroom and to any counselor who has direct contact with the student.

[Sample 1] On September 3, 1996, John Doe was placed on the SHODI list by the Arapahoe County SHODI Task Force for the following reasons:

     1.   JKD adjudicated a delinquent on 10/3/95 by pleading guilty to aggravated motor vehicle theft.

     2.   JKD adjudicated a delinquent on 4/17/96 by pleading guilty to disorderly conduct (setting a dumpster on fire).

     3.   JKD received a deferred adjudication on 10/3/95 to a theft charge (theft of almost $300 from neighbor’s home).

     4.   Charges of criminal mischief (shooting BBs with air gun at person on bus bench and near a store) and criminal trespass (breaking into vehicle) were dismissed in connection with the guilty please mentioned above.

[Sample 2] On January 30, 1996, Mary Roe was charged with aggravated robbery (theft of merchandise with use of knife), conspiracy to commit aggravated robbery, and theft. The offenses are alleged to have occurred on September 5, 1995 at a 7–11 in Englewood.
