Administrative Procedures for Sharing Disciplinary Information |
1. Principal/designee receives “disciplinary information” regarding a student in written form or requests informant providing information orally to provide written back-up.
2. Principal/designee prepares written statement (form JK-E-2) based on written information.
3. Principal/designee distributes written statement in secure envelope marked “confidential” to teachers and counselors in direct contact with student in question. Principal/designee keeps list of teachers and counselors receiving statement. Teachers and counselors instructed in statement to keep it in separate, confidential file.
4. Principal/designee sends copy of statement to student and parent (if student under 18, or if student over 18 has given written permission for parent to receive). Principal/designee places original statement, written back-up disciplinary information, and list of teachers and counselors receiving the statement in secure envelope marked “confidential—access granted only to teachers and counselors in direct contact with (name of student),” and places envelope in student’s cum file.
5. Principal/designee follows Policy JK and Regulation JK-R in the event the parent and/or student challenge the accuracy of the statement.
