Policy Name    Physical Examinations of Students

A physician’s certificate of physical examination indicating condition of the student’s health is requested in all of the following instances:

       For all students entering the District for the first time.

       Prior to each student’s entry in the third grade.

       Prior to each student’s entry in the middle school.

       Prior to each student’s entry into high school.

When schools are not provided with a physician’s certificate, it shall be the responsibility of the parents to notify the school of any impediments or impairments regarding the physical condition of their student(s).

A physician’s certificate of physical examination indicating condition of the student’s health is required in the following instance:

       Prior to any student’s participation in interscholastic athletics. Colorado High School Activities Association (CHSAA) regulations regarding time periods for physicals will prevail where applicable.

 

Exception to Physical Examination

Students may be excused from the physical examination and physician’s certificate of health (and from participation in physical education and health courses) because of religious beliefs.
